With this expertise, we are able to network with broad based community resources to provide community connected and well-coordinated supports for individuals.

I have over 4 years of case management experience. I have worked with CADI, BI, CAC & EW waivers as well as Alternative Care (AC) Program. I understand the biggest struggle that case manager have today is keeping their clients’ MA active. This is where Independent Living Partners (ILPS) can help you much as I have also struggled with this as a case manager. ILPS will ensure that individuals we serve have an active plan to keep their health insurance (MA) active at all times.
