Our services
Waiver Case Management services support individuals in service planning, provider selection, and care coordination. The goal of case management is to offer resources and collaborative guidance while empowering clients to take an active role in directing their own care. As the goals and needs of those we serve evolve, we remain committed to ensuring their supports and services adapt accordingly.
PROGRAM OVERVIEW
Independent Living Partners provides contracted case management services to individuals receiving funding through Medical Assistance and home and community-based waivers.
We offer person-centered solutions that empower individuals and families to understand their options and choose the services that best meet their needs. Our case managers take the time to clearly explain available resources and advocate for supports tailored to each person’s unique circumstances.
Case management helps individuals with disabilities access and navigate social, health, educational, vocational, and other community and natural resources. Services are guided by each person’s values, strengths, goals, and needs. Waiver case managers provide the information necessary for individuals to make informed decisions about their care.
OUR TEAM
Our team connects individuals and their families to a wide range of services, including assessments, person-centered planning, service referrals, coordination, support plan monitoring, and advocacy. We assist with both waiver-funded services and informal or community-based supports.
The Independent Living Partners case management team is composed of experienced, diverse professionals who collaborate and share knowledge to ensure the delivery of high-quality, individualized care.
