Employment services connect people to experiences that explore their interests, develop skills, and start or advance their careers. Independent Living Partners will be your partner throughout your employment journey: from Exploration to Development to Support.

Employment Services Overview

Our employment services are structured into four key stages to support individuals in achieving successful, competitive, and integrated employment.

Engage (Waiver Employment Exploration)

The Engage stage provides individuals with the opportunity to make informed decisions about employment. Participants engage in activities such as informational interviews and job shadowing to explore potential career paths.

Plan (Waiver Employment Development)

During the Plan stage, individuals identify their interests and strengths while preparing for a job search. This stage includes activities such as goal setting, benefits planning, and workplace tours to facilitate career readiness.

Find

Individuals in the Find stage have defined their work goals and are ready to pursue employment. This phase is part of Vocational Rehabilitation Services (VRS). Lifeworks, as a VRS Community Rehabilitation Provider, offers support throughout this stage of the employment journey.

Keep (Waiver Employment Support)

The Keep stage is designed to help individuals maintain and succeed in competitive integrated employment. Services provided during this phase include job coaching, skill development training, and assistance in building natural workplace supports and coworker relationships.

Eligibility for Employment Services

  • Engage, Plan, and Keep Stages: To be eligible for these phases, individuals must have a waiver for Brain Injury (BI), Community Alternative Care (CAC), Community Access for Disability Inclusion (CADI), or Developmental Disabilities (DD). Additionally, employment support services must be identified as a need in their Coordinated Services and Supports Plan (CSSP).

  • Find Stage: Eligibility for this stage is determined by Vocational Rehabilitation Services and is based on whether an individual has a disability that presents challenges in preparing for, obtaining, or maintaining employment.

How to Get Started with Waivered Employment Support

1. Complete MnChoices Assessment

Contact your county or tribal nation, also called a lead agency, to request a free MnCHOICES assessment.

2. Connect with your Case Manager

Choose your employment stage that best aligns with your current career journey.

3.Choose Independent Living Partners as your employment provider

Inform your case manager that you want to partner with Independent Living Partners.
